*If you take your Trainers or Trainers Level 1 Refresh online through the Hockey Development Centre for Ontario, you must complete the following once the course is complete:

1) Save an electronic (pdf) copy of your Trainers Certification or Refresh Certification
2) Fill out the attached Conversion form (electronically) (see below)
3) Send both, via email, to Kim Withers from the OMHA @ kim.withers@omha.net

Failure to do so will mean that the trainers certification will not be registered with Hockey Canada and we will not be able to roster you accordingly. This is an OMHA process so thanks for volunteering to help coach our kids and we appreciate your efforts in this.
